Application Form Templates
An application form collects more data than a contact form and more context than a registration form. It’s what you use when you need to evaluate someone — as a candidate, a member, a recipient, or a fit for a program. Fillin Forge’s 8 application form templates cover the common cases.

Key features for application forms
File uploads
Application forms almost always need file uploads — resumes, transcripts, portfolios, certifications. Fillin Forge’s file upload field supports:
- A maximum file size per field, set in Max file size (MB): 10 MB when you leave it empty, 100 MB at most
- Allowed file types (PDF, DOC, DOCX common)
- Private upload folder with random file names and an .htaccess rule against direct access
Multi-step format
A long application form can be split into steps with a multi-step form and a progress bar (a paid plan feature). None of the application templates is multi-step. To split a long application into steps, add Page Break fields (Pro).
Conditional logic
None of the application templates comes with conditional logic rules. You add them in the builder (conditional logic is in the free plugin). Examples of rules you could add:
- “Are you authorized to work in the country?” → if No, show visa sponsorship question
- “Years of experience?” → if 0–2, show different questions than 10+
- “Do you have references?” → if Yes, show the references repeater (Pro)
Repeater fields for references and work history
Applicants rarely have exactly 3 references or exactly 2 previous jobs. The repeater field (Pro) lets them add as many rows as they need with the + Add Row button. Fillin Forge sets no maximum number of rows. The rows hold text inputs only.
Consent checkbox
Fillin Forge has no signature field. For an “I certify the above information is true” confirmation, add a Checkbox field and mark it Required.
What happens after submission
Admin email
To get an email for each application, turn on Notify admin on submission in the Email Notifications card of the form builder and confirm the address in Notification email. In the builder a new form starts with it switched off. The email lists the value of each field and has a View submission button.
PDF of the application
The PDF is not created automatically. On the Submissions screen, each application has a PDF link that downloads that application as a PDF file. The link works for users with the manage_options capability (administrators by default), so to give an application to a hiring manager without WordPress admin access, download the PDF and send the file. See the PDF generation feature.
HR system integration
To send each application to your ATS or HR system, enter a Webhook URL in the Webhook card of the form builder. On each submission Fillin Forge posts the form ID and title, the submission ID, a timestamp and the label and value of each field that has a value to that address as JSON. The HubSpot CRM card (Pro) does less: connect HubSpot in Settings first, then Send lead to HubSpot on submission sends the applicant’s email address to HubSpot as a contact. The card has no field mapping and sets no tag, so no other field goes to the contact. Once you enter a Pipeline ID and a Stage ID, Also create a Deal adds a deal named after the form and the submission number.
Automatic follow-up email
Send an automatic confirmation to the applicant, acknowledging receipt. It is off on a new form: in the builder open the Email Notifications card, tick Send auto-reply to submitter, choose the applicant’s email field under Email field (for auto-reply address), and write the Subject and the Message. The message is your own text, so use it to set expectations for response time. The reply is sent only when that field holds a valid email address.

Application form templates are included in every version of Fillin Forge. File upload and conditional logic work in both free and paid plans; multi-step forms and repeater fields are paid-plan features.
