Booking & Appointment Templates
Fillin Forge has no category of booking templates. Booking and appointment forms sit in other categories of the gallery, and every template is in the Free plugin. These ten are the closest to a booking form. None of them has a calendar or a payment field: they ask for the day with a plain date field, and some also ask for the time with a time field or a dropdown.

Service Booking
Orders & Payments. Name, Email, Phone, Service (a dropdown: Consultation, Audit, Implementation, Training), Preferred Date, Preferred Time and Additional Notes.
Appointment Request
Healthcare. Patient Name, Phone, Email, Visit Type (a dropdown: New Patient, Follow-up, Urgent, Telehealth), Preferred Date, Preferred Time (Morning, Afternoon or Evening) and Reason for Visit.
Spa Booking
Beauty & Salon. Name, Phone, Email, Treatments (checkboxes: Massage, Facial, Body Wrap, Manicure, Pedicure), Preferred Date, Time Slot (a dropdown: 9:00 AM, 11:00 AM, 1:00 PM, 3:00 PM) and Health Concerns / Allergies.
Class Booking
Fitness & Wellness. Name, Email, Class (a dropdown: Yoga, Pilates, HIIT, Spin, Boxing), Date and Time.
Photography Booking
Photography. Name, Email, Phone, Session Type (a dropdown: Portrait, Family, Wedding, Event, Product, Headshot), Preferred Date, Location and Tell us about your vision.
Restaurant Reservation
Restaurant & Food. Name, Phone, Email, Date, Time, Number of Guests (1 to 20), Occasion (a dropdown: None, Birthday, Anniversary, Business Dinner) and Special Requests.
Ticket Booking
Events. Name, Email, Event, Date, Number of Tickets (1 to 20) and Ticket Type (a dropdown: Standard, Premium, VIP).
Trip Booking
Travel & Hospitality. Full Name, Email, Phone, Destination, Departure Date, Return Date, Number of Travelers, Budget (a dropdown: Economy, Mid-Range, Luxury) and Special Requests.
Hotel Booking
Travel & Hospitality. Guest Name, Email, Check-in, Check-out, Room Type (a dropdown: Standard, Deluxe, Suite), Guests (1 to 6) and Special Requests.
Tour Booking
Travel & Hospitality. Name, Email, Tour (a dropdown: City Tour, Nature Tour, Food Tour, Adventure Tour), Date, Number of People (1 to 15) and Preferred Language (a dropdown: English, Spanish, French, German).

Date (Calendar) field
To let the visitor pick a time slot, add the Date (Calendar) field to the form. It is in the Free plugin. Its settings have an Availability section with the first and the last hour of the working day, Slot duration (min), Advance days and the Available days of the week; the section shows 9 to 17, 30-minute slots, 30 advance days and Monday to Friday. These values are written into the field when you open its settings and save the form; a field whose settings you never open has no days set and offers every day of the week.
The field offers every slot inside those hours on those days. A date in the past, or further ahead than Advance days, is refused when the form is sent. Without Google Calendar the field does not know which slots are already taken, so it lists all of them.
Google Calendar (Pro)
With the Pro add-on, click Connect Google Account in the Google (Sheets + Calendar) card of Settings. Then open the Google Calendar card in the form builder, switch on Create calendar event on submission and pick the Calendar (Load my calendars lists yours). The form needs a Date (Calendar) field for it. With a calendar chosen, the field leaves out the slots that are busy in that calendar (if the calendar cannot be read, it shows “Could not load free times. Please try again later.” and no times), and each submission creates an event for the slot, titled by Event title template (the default is “Booking: {name}”), with the address from an Email field of the form as the guest. If the slot has become busy by the time the form is sent, or the calendar cannot be read at that moment, no event is created. If the slot is free but the event cannot be created, the event may be missing from your calendar. In each case the submission is still saved and marked “Time not confirmed” on the Submissions screen, and, when the admin notification of the form is on, the form’s notification address gets a short email.
Payment
No booking template takes a payment. To charge a deposit or the full price, add the Payment (Stripe) field (Pro) and choose Fixed amount, or Dynamic (linked to Calculation) to charge the result of a Calculation field (Pro). Enter your Stripe keys in the Stripe Payments card of Settings: payments go directly to your Stripe account.
Emails
Fillin Forge sends no reminder and no follow-up email around an appointment. It can email you each submission: switch on Notify admin on submission in the Email Notifications card of the form builder and confirm the address in Notification email. To confirm to the visitor, switch on Send auto-reply to submitter, choose the Email field (for auto-reply address) and write the Message; it is sent as you wrote it.
Compared to a dedicated booking plugin
Dedicated booking plugins (Amelia, BookingPress, etc.) offer more: staff management, resources, complex pricing, etc. Fillin Forge’s booking templates are for:
- Small businesses that don’t need a full booking system
- One-person operations (consultants, coaches, therapists)
- Simple reservations where Google Calendar is enough
- Booking forms that are one of many forms on a site
If you need staff schedules, resource allocation, or multi-location support, these templates do not cover that. If you need a booking form with available time slots, use one of them.
