Quiz & Assessment Templates
Fillin Forge has no quiz templates: none of the 168 template titles is a quiz, and no template has a Calculation field. Two templates have Assessment in the title, Health Assessment and Fitness Assessment. They collect answers; neither one scores them. A points score is possible with the Pro Calculation field, described below.
Assessment templates
Health Assessment
Healthcare. Overall health? (Excellent, Good, Fair or Poor), Exercise frequency (a dropdown), Average sleep hours (a slider from 3 to 12), Stress level (1-10) (a slider from 1 to 10) and Health concerns.
Fitness Assessment
Fitness & Wellness. Name, Age, Height, Weight, Current Activity Level (radio buttons), Current Injuries/Limitations and Goals.
Adding up numbers (Pro)
The Calculation field (Pro) adds up numbers, which is enough for a simple points score.
- Add a Calculation field with a formula such as
{question_1} + {question_2}; the builder lists the tokens under Available tokens. It shows the sum in the form as the visitor answers. - A dropdown or radio option counts as the number its value starts with, and the value of an option you type in the builder is the text you typed: an option typed as 2 points counts as 2, and one typed as Paris counts as 0. The values of the ticked boxes of a checkbox group are added up.
- That is all it does. There is no answer key, no right or wrong marking and no built-in result by score range. The Success message is plain text: it does not take placeholders such as {score}.
One question per screen (Pro)
To show a form one question at a time, switch on Conversational Mode in the form builder. It is a Pro feature, and no template turns it on, because a template carries only its fields.
