Registration Form Templates
A registration form captures who someone is and what they’re signing up for. Done right, it keeps signup short, captures the exact data you need, and feeds downstream systems (user accounts, email lists, event tracking) automatically. Fillin Forge ships 8 registration form templates for the most common use cases.
Registration form templates
User Registration
First Name, Last Name, Email, Phone, Company, Role. To create a WordPress user from the submission, turn on the User Registration card in the form builder (Pro).
Event Registration
Full Name, Email, Ticket Type, Dietary Requirements, Special Requirements.
Course Enrollment
Full Name, Email, Course, Previous Experience, Learning Goals.
Membership Signup
Full Name, Email, Phone, Date of Birth, Membership Plan, and a checkbox to agree to the terms and conditions.
Webinar Registration
Name, Email, Company, Job Title, Questions for the speaker.
Conference Registration
Full Name, Email, Organization, Badge Name, Sessions of Interest, Dietary Needs.
Workshop Signup
Name, Email, Skill Level, Bringing your own laptop?
Waitlist
Email Address (required), Name, Why are you interested? and the checkbox Notify me when available. The checkbox is required, so it has to be ticked to send the form.
Adding user accounts, email lists and payments
- User accounts (Pro). The User Registration card of the form builder creates a WordPress user after a successful submission. You choose the email field, the name field and the user role.
- Email lists (Pro). Connect your Mailchimp account once: paste the API key in the API Key field of the Mailchimp card in Fillin Forge > Settings and click Connect. Then, in the Mailchimp card of the form builder, toggle on Subscribe to Mailchimp list on submission and enter the List ID of the audience. The person is subscribed using the form’s email field.
- Payments (Pro). Add a Stripe payment field to charge for a signup.
- Emails. To get an email for each registration, turn on Notify admin on submission in the Email Notifications card and check the Notification email. The auto-reply to the registrant is off until you turn on Send auto-reply to submitter in the same card.
Data protection
Registration forms collect personal data. Submissions are stored in your own WordPress database, and you can delete a submission from the Submissions list. The templates do not add a consent checkbox or a privacy policy link for you, except the terms checkbox in Membership Signup, so add them to the form where your privacy rules ask for them.
How to use a registration form template
- Install Fillin Forge
- Go to Fillin Forge → New Form
- Scroll down to Or start from a template, click the Registration & Signup category card and pick the template that fits
- Customize fields, labels, and notifications
- Connect to user registration or Mailchimp if desired
- Save the form and embed it with
