Survey & Poll Templates
Surveys get answered when they’re short, fair, and easy to fill in. Fillin Forge’s 10 survey templates are built around that principle: meaningful questions, sensible defaults, and the option to run in conversational mode to lift completion rates on longer ones.

What makes Fillin Forge good for surveys
Rating field type
A dedicated Rating (Stars) field shows a row of stars (five by default) and saves the number the visitor picks. Five of the ten survey templates use it. Separately, General Survey and UX Research use a Range (Slider) field for a 0 to 10 or 1 to 10 score.
Conversational mode
Long surveys have terrible completion rates when presented as a single long form. Conversational mode turns any survey into a one-question-at-a-time experience that can help more people finish a long survey.
Surveys without names
Nine of the ten survey templates ask for no name, email or phone number; only Feature Request has an Email field (Your Email). That does not make a submission anonymous: Fillin Forge saves the visitor’s IP address and browser user agent with every submission, and the WordPress user ID when the visitor is logged in. There is no setting to leave them out.
Conditional branching
Use conditional logic to skip questions that don’t apply. “Did you use our support?” → if yes, show support-specific questions; if no, skip them. Shortens effective form length per user.
Rating several items
Fillin Forge has no rating matrix. To rate several items, add one Rating (Stars) field for each item. A repeater field cannot do it: its rows hold text inputs only.
Analyzing survey results
Fillin Forge shows you the answers and the numbers around them:
- The Submissions screen lists the responses of a form, and you can open each one in full
- Form Analytics (Pro) shows Views, Submissions, Conversion and Abandoned for each form over the last 7, 30 or 90 days
- Export CSV downloads the responses for deeper analysis in Excel or a data tool
Fillin Forge does not count the answers per option and does not average ratings: do that in the exported CSV.
AI submission analysis
Fillin Forge’s AI submission analysis (Pro) looks at how visitors fill in the survey and returns a score from 0 to 100 with prioritized suggestions.
