Feedback Form Templates
A good feedback form asks the right questions, respects the user’s time, and makes it easy to leave an opinion. Fillin Forge ships 8 feedback form templates covering customer feedback, product reviews, NPS, testimonials, app reviews, service ratings, course evaluations and website feedback.

Features that help feedback forms work
Conversational mode
Switch on Conversational Mode in the form builder (Pro) and the form shows one question per screen. It is off by default. See Conversational mode.
Star rating fields
The Rating (Stars) field type shows a row of stars (five by default) and saves the number the visitor picks. Seven of the eight feedback templates use it; NPS Survey uses a 0 to 10 slider instead.
Feedback without an email address
Six of the eight feedback templates have no Email field (NPS Survey, Testimonial, App Review, Service Rating, Course Evaluation and Website Feedback). Customer Feedback and Product Review have a required Email field: to collect those without an email address, remove the field or switch off its Required option in the Field Settings panel.
File upload for screenshots
None of the feedback templates has a field for screenshots. Only Testimonial has a file upload field (Your Photo, limited to .jpg, .png and .webp). To collect screenshots, add the File Upload field to a template and, in its Field Settings panel, tick Images (JPG/PNG/GIF/WebP) under Allowed file types so that only images are accepted.
Conditional follow-ups
Conditional logic (free) shows a field only when an answer matches a rule. In Website Feedback, for example, you can show a “What were you looking for?” text field only when Did you find what you were looking for? is answered No. The template has no such rule: add the Text field, open it in the Field Settings panel, tick Enable conditional logic for this field, choose Show this field and add a rule with If field set to that question, Condition set to equals and Value set to no, in lower case, the value the No option saves. This keeps the form short for visitors who found what they wanted and captures detail from those who did not.
Analyzing feedback at scale
Reading hundreds of feedback submissions manually is a waste of time. Fillin Forge’s AI submission analysis looks at how visitors fill in the feedback form and returns a score from 0 to 100 with prioritized suggestions (Pro).

Publishing testimonials on your site
The Testimonial template can be combined with Fillin Forge’s Post Submission feature (Pro). In the Post Submission card of the form builder, toggle on Create a WordPress post from submission and choose the Post Type, the Post Status (Draft, Pending Review or Published), the Title Field and the Content Field. With Draft or Pending Review, you read each testimonial before it goes live.
