This recipe builds a multi-step event registration form with conditional logic, Stripe payment, Google Sheets tracking, and a confirmation email. The form guides attendees through Personal Info, Event Selection, Dietary Restrictions, and Payment.

Prerequisites

  • Fillin Forge PRO active
  • Stripe connected in Fillin Forge > Settings (the Stripe Payments card)
  • Google connected in Fillin Forge > Settings (the Google (Sheets + Calendar) card)
  • A Google Spreadsheet created for tracking registrations

Step 1: Create the Form

  1. Go to Fillin Forge > New Form
  2. Title: “2026 Annual Conference Registration”

Step 2: Build Step 1 — Personal Information

Drag these fields into the builder:

Field TypeLabelRequired
Name (First + Last)Full NameYes
EmailEmail AddressYes
PhonePhone NumberYes
TextCompany / OrganizationNo
Page Break(no label)—

Step 3: Build Step 2 — Event Selection

After the first Page Break, add:

Field TypeLabelRequiredOptions / Config
RadioRegistration TypeYesGeneral Admission ($99), VIP ($249), Speaker ($0)
CheckboxWorkshops (Select all that apply)NoWorkshop A: Intro to AI, Workshop B: Marketing Strategy, Workshop C: Financial Planning
NumberAdditional GuestsNoMin: 0, Max: 5
Page Break(no label)—

Step 4: Build Step 3 — Dietary Restrictions

After the second Page Break, add:

Field TypeLabelRequiredOptions / Config
DropdownDietary PreferenceYesNo Restrictions, Vegetarian, Vegan, Gluten-Free, Halal, Kosher, Other
TextareaAllergy DetailsNoPlaceholder: “Please describe any food allergies…”
CheckboxT-Shirt SizeNoS, M, L, XL, XXL
Page Break(no label)—

Step 5: Add Conditional Logic

  1. Click on the Allergy Details textarea
  2. In the field panel, find the Conditional Logic section and tick Enable conditional logic for this field
  3. Set Action to Show this field. In the rule, set If field to “Dietary Preference (select)”, Condition to equals and Value to “Other”
  4. This hides the text area unless the visitor selects “Other,” keeping the form clean

Step 6: Build Step 4 — Payment

After the third Page Break, add:

Field TypeLabelConfig
CalculationRegistration TotalFormula: see below
Payment (Stripe)Pay NowAmount: linked to Calculation field

Click the Calculation field and, in its field panel, type the formula in the Formula box. Under Available tokens there are buttons for the form’s fields (one for the ID of a field and one for its label), such as {additional_guests}; click a button to put it into the formula. A token stands for the number in that field. A Radio option is read as the text you typed, and text such as “General Admission ($99)” counts as 0, so the Registration Type options cannot set the price. For example, {additional_guests} * 99 gives the cost of the extra guests. Set Prefix to $ and Decimal places to 2.

Step 7: Check the Steps

There is no multi-step switch to turn on. Fillin Forge shows a form in steps, with a progress bar and Back and Next buttons, when it contains Page Break fields (Pro). The three Page Breaks you added split this form into four steps. The steps have no names: a Page Break has no label to fill in, and the progress bar shows the step number, for example 1 / 4. Save the form.

Step 8: Connect Google Sheets

  1. Find the Google Sheets card in the builder
  2. Toggle on Append submission row to Google Sheet
  3. Under Spreadsheet, click Choose spreadsheet and pick your tracking spreadsheet. The card shows the tab that will be used: the first tab of the sheet
  4. Fillin Forge does not read a header row. Each row it adds starts with the submission ID and the date and time, followed by one cell for each field of the form that takes an answer, in the order of the form, so you can add headers for your own reference

Step 9: Set Up Confirmation Email

  1. Find the Email Notifications card in the builder
  2. Toggle on Notify admin on submission and enter your event coordinator’s email in Notification email
  3. Toggle on Send auto-reply to submitter, then under Email field (for auto-reply address) select the Email Address field
  4. Set the Subject and the Message:
– Subject: “Registration Confirmed — 2026 Annual Conference”

– Message: Thank the attendee, confirm that the registration was received, and mention any next steps. The message is sent as you write it, so it cannot include the attendee’s name, registration type or workshops

Step 10: Test the Full Flow

  1. Embed the form on your registration page
  2. Walk through all four steps using Stripe test mode (card: 4242 4242 4242 4242)
  3. Verify the submission is saved, the payment appears in Stripe test dashboard, a new row appears in Google Sheets, and both notification emails arrive
